**Free Sperm Cryopreservation Storage for 1 Year for Men Having Babies Members
Offer includes a free year of cryostorage for sperm for self-pay packages or non-covered services. Valid for new patients only and is non-transferable. Non-covered services are defined as services in which a patient’s plan does not have coverage. To qualify for one year of free sperm cryostorage a physician consultation and semen testing must be completed through Fertility Centers of Illinois. This discount does not cover semen analysis, FDA testing, medication, laboratory work, genetic testing, co-pays, co-insurance, deductibles, or other insurance-based out-of-pocket costs.
