Patient Portal
Our Patient Portal is a web-based system that uses real-time technology to provide access to all of your treatment needs. Patients can schedule appointments, view lab results and medicines, and even message directly with our practice. The Patient Portal puts every element of your treatment plan at your fingertips. Please review our after-hours policy regarding contact with your care team outside of clinic hours.
Starting in July 2024, we are excited to introduce an updated process for accessing our patient portal to better serve you. If you are beginning a new treatment cycle, please use this link to access the patient portal.*
For those who began their treatment cycle prior to July 2024, and for Dr. Kaplan’s patients, you will continue to use this link.
As we work towards transitioning everyone to the new portal, we appreciate your patience and cooperation. Over the next few months, all patients will gradually shift to the new portal link to enhance your experience. Please call your local clinic if you have any questions accessing your patient portal.

Online Bill Pay
Easily pay deposits and statement balances online.
By clicking the link below, you will be sent to third-party website, where you can safely and securely make online payments. Cryopreservation or Annual Storage Fee
For patients who are paying for cryostorage of embryos, eggs, and/or sperm, and/or monthly storage fees, please make payments through the Embryo Options Portal. Not currently enrolled with Embryo Options? Enroll now.
By Phone
Simply call us at 847-729-2188 anytime during business hours and we can verify the payment due, set up payment arrangements, and take your credit card information over the phone. We accept Visa, MasterCard, American Express and Discover.
By Mail
To mail your payment to us, simply send us the top portion of your billing statement along with a personal check or your credit card information authorizing payment.
Medical Records
To obtain a copy of your medical records, please return a completed copy of the medical records authorization form. If you have not yet completed an authorization form, please download and print the form, download it here. You will need to return the authorization form to us by emailing your request to fcimedicalrecords@fcionline.com or by printing and faxing the form to 847-486-7074. If the request is Urgent, please only Email your request to fcimedicalrecords@fcionline.com.
The turnaround time to receive copies of medical records is 30 days from the date the request is received. We do provide expedited medical records requests at an additional charge. For expedited requests, please email the medical records department at fcimedicalrecords@fcionline.com with the subject line, “Expedited” and we will do our best to accommodate your specific needs.
Fees
$35.00 for an electronic copy
$50.00 for a jump drive (or USB/flash drive
$75.00 for a physical copy
$25.00 expedited fee for records needed within one week of request
Payment and Timeline for Medical Records
The normal turnaround time to receive copies of records is 30 days. Once we are ready to process the request, we will send you a payment link. We kindly ask that you do not pay any fees without us reaching out to you.
Your Medical Records and Your Privacy
Ensuring patient confidentiality is a top concern of Fertility Centers of Illinois. To learn more about this, read our Notice of Privacy Practices.
Fertility Centers of Illinois maintains patient records in accordance with regulatory standards. These standards do not require that all patient records be permanently maintained. Depending on the date and type of service, patient records and films may no longer be available for access and duplication.
COVID-19 Policy
Before you come to your appointment, make sure you have no COVID-19 symptoms (cough, fever or chills, shortness of breath, fatigue, sore throat, loss of taste or smell, gastrointestinal symptoms). Face masks are encouraged when visiting an FCI clinic. Due to the sensitivity of our IVF care, we will still require masks in IVF areas at our Chicago River North and Highland Park offices.
