Patient Financial Responsibility Notice Insurance Company Change Year-to-Year

Often a new year brings changes with personal insurance provider selections. This signed form below will be required to be received by our scheduling staff by December 8, 2021 for any patient treatment services scheduled between the dates of December 13, 2021 and December 31, 2021.

Fertility Medication Resources

View injection techniques on Freedom Fertility Pharmacy’s MedTEACH platform. Nearly all prescribed injectables are listed. Simply view the video for each medication that your nurse has ordered for you.

Patient Education

We are dedicated to providing you with all of the support you need. Our fertility experts, financial specialists, on-staff clinical behavioral health specialists, and alternative health care professionals host events and webinars to keep you informed.

You have an entire team committed to your physical and emotional well-being along your journey to parenthood. Our seminars, workshops, and webinars are open to the community. You do not need to be our patient to attend. They are all entirely free unless otherwise noted.
