Your Path Forward
Every great journey starts with a single step
Beginning this journey is a big decision, and we’re honored to be your trusted partner.
Your New Patient Journey
Your journey begins with a conversation with your physician to explore your goals, vision, and dreams of building a family. Together, you’ll create a personalized action plan tailored to give you the best chance of success. Miracles don’t happen overnight, but we’re here to support your pace — whether you’re eager to move quickly or prefer to take things slow.

- Schedule Appointment: It all starts with a conversation. Prepare your questions and goals to discuss with your physician. Appointments can be in-person or virtual.
- Submit New Patient Paperwork: Complete your new patient forms at least 3 days before your consult. In Phreesia – our secure, HIPAA-compliant system – confirm your appointment, fill out your forms, and upload your insurance, ID and credit card. Then, check your email for a link to create your patient portal account (and partner account, if needed) to submit your medical intake form and upload medical records.
- Insurance Verification: About two days before your consult, check your patient portal for a welcome message from our Insurance Benefit Verifications team with detailed benefits information. A copy is also available in the Files section under the navigation menu. If you’re a self-pay patient, we’ll include an overview of common work-up and treatment costs to help you prepare.
- Physician Consult: Your first consultation—whether in person or online—provides an opportunity to review your family building goals, discuss your medical history, and understand what fertility testing and next steps are right for you.
- Financial Educator Check-In: About two days after your consult, your Financial Educator will reach out to introduce themselves, give you their direct phone number, and walk you through your insurance coverage, fertility benefits for diagnostics, and any necessary requirements such as pre-authorization requirements or self-pay costs. Optional: Schedule a 1:1 Session with your Financial Educator.

- Fertility Testing – Female: Based on your goals and medical history, your physician will recommend blood hormone testing (FSH, LH, AMH, and E2) and a vaginal ultrasound during the first few days of your menstrual cycle to assess ovarian reserve and egg supply. Please call on Day 1 to schedule your appointment for day 2-4 of your menstrual cycle, or call on Monday if your period begins on a weekend. Additional tests, such as an HSG or SIS, may also be recommended to evaluate the uterine cavity and fallopian tubes. Fertility Testing is scheduled Monday – Friday and can be done at numerous FCI clinics.
- Tip: Fertility Testing is available at numerous FCI clinics, so you can schedule your appointment at the location convenient for you. Appointments are required.
- Fertility Testing – Male: A semen analysis measures the health and quality of sperm. Specimens can be collected at home using an FCI collection kit or in a private collection room at select offices. Appointments are required and scheduled Monday–Friday at numerous FCI clinic locations. Your care team will provide specific collection instructions and guidelines prior to testing. Additional testing may also be recommended based on the results. Optional: FCI has a Reproductive Urologist, Dr. Samuel Ohlander, who provides comprehensive urology care focused on identifying and treating the root causes of male infertility.
- Genetic Carrier Screening: Genetic carrier screening is a blood test that checks if you, your partner, or a donor carries genes for inherited conditions that could be passed on to future children. Ask your care team about test options. Insurance coverage varies.
- Note: Preimplantation Genetic Testing (PGT): Preimplantation Genetic Testing (PGT) is a separate test performed on embryos for genetic or chromosomal abnormalities.
- Follow-Up Physician Consult: During this important follow-up appointment you will review the results of your fertility testing and any other tests. You could approve the treatment plan or align on next steps.

Approve Treatment Plan
If you and your physician decide to move forward with treatment plan, the preparation for treatment steps will begin. Your care team will guide you through each step so you feel informed and supported along the way.
At this point you will need to select IUI, IVF or Ovulation Induction. If doing IVF, you will need to determine if you are doing Preimplantation Genetic Testing (PGT) and a Frozen Embryo Transfer (FET) or Fresh Embryo Transfer.

Align on Next Steps
If you are not ready to start treatment, this is a good time to discuss any questions, concerns, or timelines with your physician so you can align on the next steps. Your physician or care team could provide additional resources to help you make informed decisions.
- Prepare for Treatment Steps: Once you and your doctor approve the treatment plan, the preparation for treatment steps begin which include medications, consents and financial approval. We’ll guide you through each one so you feel ready and supported every step of the way.
*Timing and process may vary based on individual circumstances. All timelines are calculated in business days (weekdays only, excluding holidays).
